Cardiothoracic Step-Down Unit at Columbia
Columbia’s Cardiothoracic Step-Down Unit provides an unmatched level of supervision and attention for patients as they recover from surgery.
The Cardiothoracic Step-Down Unit (SDU) at Columbia University Irving Medical Center/NewYork-Presbyterian Hospital (CUIMC/NYP) provides an unmatched level of supervision and attention for patients as they recover from surgery. Our unique model for managing postoperative care ensures that every patient has the support they need as they begin their recovery.
- We are the only service in the region managed by a dedicated on-site cardiothoracic surgeon, providing a high level of expertise when evaluating postsurgical patients.
- Our team of specially trained physician assistants and nurses provide extensive supervision and support, with far more team members than typical SDUs.
- Our model allows us to react to emergencies faster and more efficiently, meaning we can identify and address issues like infection sooner and reduce the risk of additional complications.
What is a Step-Down Unit?
The stepdown unit (SDU) is an essential part of any surgical recovery, helping to begin the recovery process while monitoring patients for complications. After your surgery, you’ll spend time in the intensive care unit (ICU). Once you are in stable condition, you’ll head to the SDU.
While in the SDU, you will be encouraged to move and continue the respiratory and physical therapy you may have started in the ICU.
This intermediate level of care is crucial during the period following surgery, both to provide rapid responses to life-threatening issues should they arise and to ensure that patients are safe to transition towards recovery and release.

Program Highlights
A Care Team Led by an Experienced Cardiothoracic Surgeon
Our approach to SDU care begins with our leadership. Syed T. Raza, MD, serves as Medical Director for our care team. While most programs have a surgeon on call in case of emergencies, Columbia is unique in having a dedicated cardiothoracic surgeon be part of the on-site patient coverage.
Extensive Support From a Deep Team
Dr. Raza leads a team of more than 20 physician assistants that see to each patient, providing exception coverage at all times. Our PAs are located right on the SDU floor to better provide immediate care when needed.
Better Coverage Means Better Outcomes
By maintaining a large and well-supported care team, our SDU has reduced complications like arrhythmia, atrial fibrillation, or post-surgical infections and improved outcomes for our patients.
Coordinated Cardiac and Neurovascular Support
In the rare event of a serious complication, our SDU team works directly with the rapid response team and cardiothoracic and neurosurgical units to act quickly and decisively. When seconds count, our extensive SDU coverage makes an enormous difference.
